In addition to good technical defense systems, we need to continually educate the user with effective security awareness programs. While the user is responsible, I see many organizations shrugging off the importance of a Security Awareness program for their users. So IMHO, corporate management can be blamed in these cases as well as they fail to properly educate their professionals in the best practices for security.
